As part of the Hertfordshire County Council commissioned transition project, the Herts for Learning Early Years team present a briefing presentation to share details of the project, the transition toolkit and the transition level of need pathway. The overall focus of the project is to ensure a smooth transition for all children, including those at risk of underachievement.

    The briefing is for early years practitioners working in group settings including school Nursery and Reception classes, nursery schools, preschools and day nurseries in the PVI sector.  

    The briefing will provide information on the transition level of need pathway and tools developed as part of the project. It will be important that practitioners gain an understanding of how this can support early identification of those children with potential barriers to learning and help enable them to experience a smooth transition to school. Including the introduction of the level of need criterion and tool to identify children that will require support with their transition into school.
